Unfortunately accidents do happen, and when they do it is important to investigate to understand why happened and why their consequences were as sever as they were.
Then we need to learn from them and to take actions to seek to avoid a repetition. Investigation and learning from accidents, incidents and near misses (whether they be safety, environmental or operational) is a key part of any organisation’s management systems. It is a regulatory requirement for major hazard facilities. Good investigation goes beyond immediate and basic technical causes to examine human actions and identify root causes, barrier failures and management system deficiencies.
